58例乳腺癌患者ABVS表现与ER、PR及HER-2的价值研究

Correlation between ABVS and ER,PR and HER-2 in 58 breast cancer patients

目的探讨自动乳腺全容积成像(ABVS)特征与雌激素受体(ER)、孕激素受体(PR)及人类表皮生长因子受体2(HER-2)表达的价值研究,为乳腺癌患者的内分泌治疗、预后及评估提供一定帮助。方法回顾性分析经病理证实的58例患者共59个乳腺恶性病灶的术前ABVS和传统手段超声图像特征,分别观察ABVS及两者相结合的临床诊断效果。应用免疫组化法对ER、PR及HER-2进行检测,同时考察其基因表达与超声影像学表现之间的关系。结果58例乳腺癌患者59个乳腺恶性病灶中ER、PR及HER-2的阳性表达分别为61.0%、64.4%、35.6%。ER、PR的阳性表达与冠状面汇聚征和边缘毛刺征相关(P<0.05)。HER-2的阳性表达与腋窝淋巴结转移、ABVS肿块内微钙化和形态不规则相关(P<0.05)。结论分子生物学指标ER、PR及HER-2的表达与乳腺癌ABVS超声征象冠状面汇聚征、边缘毛刺征、肿块内微钙化、形态不规则及腋窝淋巴结转移等有一定的相关性。

Abstract:

【Objective】To investigate the relationship between the characteristics of automatic breast volume imaging(ABVS)and the expression of ER,PR and HER-2,and to provide some help for endocrine therapy,prognosis and evaluation of patients with breast cancer.【Methods】Fifty-nine malignant breast lesions in 58 patients with pathologically confirmed breast lesions were retrospectively analyzed for preoperative ABVS and conventional sonographic features,and the clinical diagnostic effects of both ABVS and conventional sonography were observed.ER,PR and HER-2 were detected by immunohistochemistry,and the relationship between gene expression and ultrasonic imaging was investigated.【Results】The positive expression of ER,PR and HER-2 were 61.0%,64.4%and 35.6%in 59 malignant breast lesions of 58 breast cancer patients.The positive expression of ER and PR was correlated with coronary convergence sign and marginal burr sign(P<0.05).The positive expression of HER-2 was correlated with axillary lymph node metastasis,microcalcification in ABVS mass and irregular shape(P<0.05).【Conclusion】The expressions of ER,PR and HER-2 in breast cancer were correlated with the coronary convergence sign,marginal burr sign,microcalcification,irregular shape and axillary lymph node metastasis of ABVS.
