胰腺囊性肿瘤7例诊治分析

Diagnosis and treatment of pancreatic cystic tumors (report of 7 cases)

摘要:

目的总结胰腺囊性肿瘤的诊治经验.方法对1990年7月～2004年7月7例临床确诊的胰腺囊性肿瘤进行回顾性研究.结果 7例病人中,浆液性囊腺瘤3例,粘液性囊腺瘤1例,囊腺癌3例.1例位于胰头,6例病人均位于胰腺体尾部.全部病人均接受了手术治疗:3例行胰腺体尾加肿块切除术,2例浆液性囊腺瘤行囊肿剥除术,1例行胰十二指肠切除术,1例由于肿瘤广泛转移而仅行活检术.全组无围手术期死亡,除1例术后半年死亡外,其余愈后良好.结论临床中胰腺囊性肿瘤常易误诊为假性囊肿,病史询问,影象学检查,囊液分析以及组织活检有助于明确诊断.手术切除是唯一的有效治疗方法,预后满意.

Abstract:

Objective: To report clinical experience of the diagnosis and treatment in cystic tumor of 7 cases. Methods: We retrospectively studied 7 patients with pathologically confirmed cystic tumors of pancreas from Jul 1990 to Jul 2004. Results: Of all the 7 patients, 3 were serous cystadenoma,1 was mucinous cystadenoma, 3 were cystadenocarcinoma. Besides 1 lesion was located in the pancreatic head, 6 invaded the bodytails of the pancreas. The 7 patients were treated by the duodenopancreatectomy (1), distal pancreatectomy (3), enucleation(2), biopsy (1). There was no death occurred perioperatively. Besides 1 case of cystadenocarcinoma died within 6 months, others had desirable effect. Conclusions: Cystic tumors of pancreas are often misdiagnosed for pseudocyst in clinic practice. Detailed history, imaging examination, cyst fluid analysis and biopsy histological examination are main diagnostic workups. The operation is worth recommending which can identify diagnosis and offer a good prognosis.
